fix(gen): this-adjust 失败静默跳过改 fail-fast

非第一父类方法调用时，this-adjust 任一步失败（父类/子类 struct 未注册、
首字段缺失、字段查找失败）此前静默用未调整的 obj_ptr 当 self -> 方法按
自身布局读字段读到垃圾（lookup_reexport strcmp 概率崩溃根因候选）。
新增第一父类判定（前缀对齐 delta==0 免调整），其余失败路径 fatal_error
报出方法/定义类/接收类。
