dir_read_entry 里 win_read 失败原本静默返回 FR_DISK_ERR：错误虽有上抛，但没有任何现场，上层只能看到 find end res=1（当成找不到文件）或把 win[] 里的残留字节当垃圾目录项（name=ELF、attr=00）解析，无法判断根因是读盘失败。现改为显式打印 [f32] dir read failed sec/off/dirsec/res 后再返回 FR_DISK_ERR。
