refactor(tpv): INTPTR/UINTPTR 去硬编码，依赖 stdint.vp CTypedef 声明

- map_t_type 删除 INTPTR/UINTPTR = Ptr(i32) 硬编码（与 stdint.vp L10/L14 声明重复）

- INTPTR = t.CInt|t.CPtr、UINTPTR = UINT|t.CPtr 由 CTypedef 动态查表解析（L1377 先于 map_t_type 兜底）

- stdint.vp 成为单一事实源；语义不变（INTPTR 是 int* 历史命名，非 C 标准 intptr_t 整数）
