debug(phase2): 打印 list[ptr] 构造后内部状态定位 append 崩溃

崩溃仍在 list[ptr].append 的 pool.alloc（0xf8→0x128 偏移变化说明
elem_size 修复生效），需区分 __pool__ 未初始化（__init__ 未调/写错位）
与 MemBuddy 内部损坏。新增 [LLC-DBG2] 打印 pool/count/cap/elem/data。
